Title : Population health, public health and the social determinants of health: The state of the science
Abstract:
Health outcomes are influenced by far more than healthcare services alone. A growing body of evidence demonstrates that social, economic, environmental, and structural factors significantly affect health status, quality of life, and longevity. Population health, public health, and the social determinants of health (SDOH) provide complementary frameworks for understanding and addressing these influences while advancing health equity and improving outcomes across communities. This presentation examines the current state of the science related to population health, public health, and SDOH, highlighting their interconnected roles in promoting wellness, preventing disease, and reducing disparities. Participants will explore key determinants of health, including economic stability, education, healthcare access, neighborhood and environmental conditions, and social and community factors. Current research regarding the impact of these determinants on health outcomes, healthcare utilization, and health inequities will be reviewed. The session will also discuss emerging evidence on structural determinants of health, health equity, and the influence of policy on population well-being. Particular attention will be given to the expanding role of healthcare organizations and clinicians in identifying and addressing social needs through screening initiatives, community partnerships, care coordination, and value-based care models. Examples of successful interventions and innovative approaches will illustrate how healthcare systems can collaborate across sectors to improve outcomes and address root causes of poor health. By synthesizing contemporary evidence and best practices, this presentation will provide attendees with practical insights to strengthen population-focused strategies, address social determinants, and support equitable, sustainable improvements in health outcomes.
