Population Health Managers

Population Health Managers coordinate strategies to improve health outcomes for defined populations using data-driven and collaborative approaches. They analyze population health data to identify risks, stratify populations, and prioritize interventions that prevent disease and manage chronic conditions. In public health and healthcare systems, population health managers integrate clinical care, public health services, and social supports to address social determinants of health. Their responsibilities include program planning, care coordination, performance monitoring, and evaluation of outcomes. By aligning resources with population needs, they help reduce avoidable hospitalizations, improve quality of care, and control costs. Population health managers work closely with clinicians, community organizations, and policymakers to implement sustainable, equitable interventions that improve overall population wellbeing.
